Animal Eye Associates recommends that owners of breeding and service dogs schedule certified eye screening exams to ensure their dogs are free from hereditary eye conditions. The Orthopedic Foundation for Animals (OFA) oversees the certification process once the exam and forms are completed by one of our board-certified veterinary ophthalmologists.

It’s important to note that the OFA eye certification exam is a screening only—it does not include diagnostic testing and is not designed to monitor or treat existing eye disease. After your appointment, you’ll receive a completed OFA form, which must be submitted directly to the OFA for certification at their discretion. Certifications are valid for one year and should be renewed annually.

On the day of your appointment with our dog ophthalmologist, please bring your dog’s information with you to complete OFA forms, including:

  • Registration number
  • Owner’s name and contact information
  • Dog’s registered name, date of birth, sex, breed, and — if applicable — their permanent ID# (ie; microchip or tattoo).

Service Dog Eye Exam Event

The Animal Eye Associates is proud to participate in the National Service Animal Eye Exam event every May. This annual program provides free eye exams for qualified service and working animals, thanks to the volunteer efforts of board-certified veterinary ophthalmologists across the U.S., Canada, and Puerto Rico. Each year, nearly 8,000 animals benefit from these screenings, helping protect the vision of those who dedicate their lives to serving others. Registration is open from April 1–30 annually!
